Where it all comes together

To the left are the permits and applications the city uses to conduct business.  Before filling out any of the forms, please be sure you have the correct one.  Providing the wrong form may delay the approval of your permit or application.  All forms are provided in PDF format and are fillable on-line.  If you have any questions, feel free to contact us.

Building Permit

Building permits are required for new construction and additions to existing construction. The Building Permit must be filled out and signed by a master electrician (if applicable), a master plumber (if applicable) and the applicant.

Commercial Alarm Application

The Commercial Alarm Application is for all businesses that have a burglar alarm, fire alarm, medical alarm or any combination thereof. The application must be signed by the applicant.

Commercial Utility Application

The Commercial Utility Application is for all businesses that require water utilities. Applicant must sign the application. A "Guide to City Services" is included with this application.

First Pay Authorization Form

The First Pay Authorization Form is an application for those wishing to have their utility bill automatically withdrawn from their checking or savings account. This is a convenience the city offers to all customers and is not required in order to have utility service. Applicant must sign the application.

Manufactured Home Permit

The Manufactured Home Permit is required for all new or used manufactured homes brought into the city. Manufactured homes brought into the city can not be any older than 7 years from the date of manufacture. The retailer/installer must sign this permit.

Manufactured Home Utility Application

The Manufactured Home Utility Application is for all manufactured homes that require water utilities. Applicants must sign the application. A "Guide to City Services" is included with this application.

Residential Alarm Application

The Residential Alarm Application is for all residences that have a burglar alarm, fire alarm, medical alarm or any combination thereof. The application must be signed by the applicant.

Residential Utility Application

The Residential Utility Application is for all businesses that require water utilities. Applicant must sign the application. A "Guide to City Services" is included with this application.

Roll-off Agreement Form (demo)

This Roll-off Agreement form is used for renting a roll-off container for the collection of demolition and construction waste. This form must be signed.

Roll-off Agreement Form (limbs)

This Roll-off Agreement Form is used for renting a roll-off container for the collection of limbs. This form must be signed.

Specific Use Permit Application

The Specific Use Permit Application is used to request a variance, such as when a business is operating in non-conforming use. This application must be signed.

Water Termination Form

The Water Termination Form is for any residence or business that is requesting to terminate their water service. Application must be signed by the applicant.

Zoning Request Application

The Zoning Request Application is used for requesting a zoning change, such as Open Ag to Residential. This is simply a request for the application to be brought up before the Planning and Zoning Board.
